Annalise McKenna Maison-Chavez, passed away Saturday, February 18, 2012 with her family by her side. She was born on November 18, 2009 in Louisville, CO. to Mother Erin Maison-Everhart and Father Erik Chavez. She is survived by her mother, sister Charlize, maternal grandfather Donald Maison Jr., Great Grandmother and Aunts Megan, Stacy and Samantha. Annalise is also survived by her father, paternal grandparents Jeanie and Eddie Chavez, Aunts Elisa, Tiffany, Connie, Gail, uncles Aaron and Richard and cousins. Services will be held Saturday, February 25th at 2pm at Calvary Church in Broomfield CO. Interment is TBD. In lieu of flowers, donations in her name are requested to Children’s Hospital of Colorado.
