Phillip Carl Fornash Sr., 78, of Worthville, KY passed away Wednesday, November 14, 2018 at University Hospital in Cincinnati, OH. He was preceded in death by his parents, Beryl and Alice Fornash, and his brother, Don Fornash. Phillip is survived by his loving wife, Annette Fornash, his beloved children: Debbie Burke (Ted Talbot), and Phillip Fornash Jr., his sister: Wanda Arlinghaus (John), 4 grandchildren, and his dear fur babies. Phillip’s wishes were to be cremated, and services for Phillip will be handled in private at the convenience of his family.
